Guide to the APNIC Second Opinion Request Form (text version)

Before you start

The APNIC Second Opinion Request Form will lead LIRs through the steps required to seek APNIC's approval to:

  • make assignments or sub-allocations of IPv4 address space that exceed the LIR's Assignment Window
  • assign multiple or additional IPv6 /48s to a single customer

Important: for each type of request, you must submit a separate request form.

All second opinion requests must include the following completed templates:

  • Requestor template
  • Second opinion template

Depending on the type of second request you are submitting, complete ONE of the following templates ONLY:

  • IPv4 assignment template OR
  • IPv6 assignment template OR
  • Sub-allocation template

The text form is intended to be completed in text and submitted by email to APNIC. Please note that it may be parsed by machine, before it is passed on to the APNIC hostmasters. Therefore, you must ensure that:

  • your account-name, enclosed in square brackets, is included in the subject line. For example,
  • [SPARKYNET-MY]
  • your email is sent in plain ASCII text only
  • you do not modify any of the template lines starting with #[
  • you do not modify any of the attribute names in the templates.
