Guide to the APNIC Portable Assignment Request Form (text version)
Before you start
This guide accompanies the APNIC Portable Assignment Request Form and will lead you through the steps required to request IPv4 or IPv6 portable assignments for:

Critical Infrastructure |
IPv4 or IPv6 |
Important: for each type of request, you must submit a separate request form.
If you do not yet have an APNIC account name, please contact admin@apnic.net before completing this form.

All requests for portable assignments must include the following completed templates:
- Requestor template
- Portable assignment template
- Network template
- Technical template
Depending on the type of portable assignment you are requesting, complete ONE of the following templates ONLY:
- Multihoming OR
- Internet Exchange Point OR
- Critical Infrastructure
Then, submit the completed templates in text by email to: hostmaster@apnic.net
Please note that it will first be parsed by machine, before it is passed on to the APNIC hostmasters. Therefore, you must ensure that:
- your account-name, enclosed in square brackets, is included in the subject line. For example,
[SPARKYNET-MY]
- your email is sent in plain ASCII text only
- you do not modify any of the template lines starting with #[
- you do not modify any of the attribute names in the templates.
